1. Obtain a Petition for Modification or Waiver of the Graduate School Academic and Administrative Policies form (see left column under "Related Links") from:

• the Graduate School, 200 Boone Hall or
  
  
• the Office of Records and Registration, 303 Pierce Hall or
  
  

2. Complete the form by:

• signing it
  
  
• indicating what program change was made and how you were able to complete program work
  
  
• indicating why the 10 hour rule should be waived
  
  
• obtaining the graduate program coordinator/advisor's signature
  

3. Return form:

• to the Graduate School, 200 Boone Hall (fax 734-487-0050)
  
  
• prior to application for graduation.
